  • If the applicant relocates or transfers, please modify or cancel the electronic receipt application information immediately.
  • The content sent by the Electronic Receipt Service is for reference only for the applicant's payment of fees, and is not used for other purposes. In order to protect the security and confidentiality of the use of personal data, the applicant agrees and guarantees that the service will not be used to infringe upon the rights and interests of others or engage in illegal acts.

Personal Data Collection, Processing and Utilization Statement and Consent
新竹瓦斯股份有限公司（以下簡稱本公司）受理及執行天然氣供應業務所需，茲依據個人資料保護法（以下稱個資法）第8條規定，向台端告知下列事項：
一、 蒐集之目的：天然氣裝置之新設及增設、管線抽換及廢除、計費抄表、天然氣費及裝置工程費之收費退費、氣費保證金退費或過戶、各項裝拆驗表之表務作業、緊急搶修、定期安全檢查，及為辦理以上業務之通訊聯繫、用戶資料庫管理、公司內部控制及稽核等業務，而蒐集、處理或利用台端之個人資料。
二、 蒐集之個人資料類別：姓名、出生年月日、身分證統一編號、通訊地址、聯絡電話、電子郵件信箱、金融機構帳號等資訊。
三、 個人資料利用之期間、地區、對象及方式：
（一）期間：本公司經營天然氣供應業務期間，或依相關法令本公司執行業務所必需之保存期間。
（二）地區：僅限於中華民國台灣境內。
（三）對象：本公司、各服務中心（站）、委外廠商，及公用天然氣事業主管機關或依法有調查權之司法機關。
（四）方式：以電子檔案或紙本檔案形式。
四、 依個資法第3條規定，台端就本公司保有台端之個人資料得行使下列權利：
（一）得向本公司查詢、請求閱覽或請求製給複製本。
（二）得向本公司請求補充或更正，惟依法台端應為適當之釋明。
（三）得向本公司請求停止蒐集、處理或利用及請求刪除，惟依個資法第11條第3項但書因執行業務所必須者，得不依台端請求為之。
（四）方式：以電子檔案或紙本檔案形式。
五、 台端不提供相關個人資料所致權益影響： 台端保有提供個人資料予本公司處理利用之權利，惟若拒絕提供個人資料、或提供不完整不正確之個人資料，或提供後向本公司請求刪除部分或全部個人資料時，本公司將無法進行必要之審核及處理作業，致無法提供台端相關服務。
六、 本告知聲明暨同意事項如有未盡事宜，依個資法或相關之法令規定辦理。

經貴公司向本人告知上開事項內容，本人已充分瞭解貴公司蒐集、處理及利用本人個人資料之目的、用途及相關權益，並同意貴公司在上述蒐集目的範圍內處理及利用本人之個人資料。
